Registration forms are available in PDF format. If you have the Adobe Acrobat reader installed on your system, you can now download and print a picture-perfect copy of those forms. The reader software is free to download and use.

TRIPLETS/QUADS/QUINTS:
There is no special registration form for multiples other than twins. If you
need additional room, please print another copy of this form or use an another
sheet of paper. The registration fees for multiples other than twins
will be as follows:
Triplets: $22.50 per set
Quadruplets: $30.00 per set
Quintuplets: $37.50 per set
Here's what you get for a $15.00 registration fee. ($20 on-site)
Non-Twins (siblings, parents, etc.): One of the purposes of registration is to get an accurate count of the number of twins attending. This is important for planning as well as promotion. Non-multiples can simply pay a nominal admission fee at the gate (currently $3).
